This is especially important for medications with a narrow therapeutic range—where the difference between an effective dose and a harmful one is small. Even minor changes in how the body processes these drugs can have major consequences.

Fat, muscle, and water—how body composition shifts

A younger body typically contains more muscle and water, while an older body tends to have more fat. This shift affects how medication is distributed. Fat-soluble drugs—such as certain sleep aids or pain relievers—can accumulate in fatty tissue and be released slowly over time. That can prolong their effect but also increase the risk of side effects.

The liver plays a central role in metabolizing many medications, but its capacity declines with age. This means some drugs remain active in the body longer, potentially intensifying their effects. The kidneys, which filter waste and drug byproducts, also lose function over time—often without obvious symptoms. Routine blood tests can help detect these changes.

That’s why it’s important for healthcare providers to regularly check liver and kidney function in older adults, especially those taking multiple medications. Adjusting doses can make treatment both safer and more effective.

Many older Americans take several medications at once—prescription drugs, over-the-counter remedies, and dietary supplements. This increases the risk of drug interactions. Some combinations can amplify effects, while others can reduce effectiveness or cause unexpected side effects.

A common example is blood thinners, which can interact with pain relievers or herbal supplements. Keeping an up-to-date list of all medications—including vitamins and natural products—and sharing it with your doctor or pharmacist helps prevent dangerous interactions.

  • Stay hydrated and eat well – dehydration and poor nutrition can affect how drugs work.

Finding the right balance

There’s no one-size-fits-all approach to medication in older age. Every body is different, and age is only one factor. Genetics, lifestyle, diet, and other health conditions all play a role. The goal is to find the right balance—where medication helps without putting unnecessary strain on the body.
